Case Management

Changing Seasons Home Care provides our clients with medical case management services. We act as a liaison between medical providers, clients and families when necessary. Our staff help identify problems and connect our clients with the appropriate resources they need to get the best care possible. When we complete the initial paperwork at the start of care, we review all medications and confirm that the client is on the correct medications with their physicians. As people age and their health begins to deteriorate, many people need multiple medications. It can be confusing to manage all the medications and be sure your loved one is taking them correctly. We provide added assurance that our clients are, in fact, getting the treatments they require. We work in collaboration with our client’s physicians to report any concerns and seek treatment when indicated. It is reassuring to know that your family member is being well cared for when you are unable to be with them.
